The heims on the radius arms go into tabs off the frame. These tabs can be welded to the frame or made into a bracket that welds or bolts to the frame. A common spot to have these tabs is near the mounting area for the transmission cross mount. There is lots of information and pictures on this!

That bottom part is pretty bad. I recommend cutting off and doing new templates out of cardboard. Draw a line along the center of the open tube. This line can be used as a reference point to make the templates. I can do a drawing for ya if you'd like. Others here could provide help too...
